It is a one year evidence based (incorporating DH’s PCO frameworks, UK NICE guidelines & etc) , structured ( scheduling focused FU sessions with clear milestones), personalized (customizing progress) with risks mitigation (achieving optimal overall wellness).
It is delivered by an experienced Wellness Master in collaboration with health professionals.
The program is tailored toward 4 groups
It refers to stakeholders surrounding and contributing to wellness of the client including primary care doctor(s), nurse counsellors, spouse and family members, co-workers and peers, allied medical specialists, nutritionist, psychologists, home care personnel, significant others and volunteers
